You do not need a formal referral to book an appointment for consultation. We welcome all patients and we accept all referrals weather from dentists, physicians, other health professions, patients, friends or family members. Over the telephone we can discuss your concerns and help you determine if a consultation with our specialists would be appropriate for your needs.
To simplify the referral process you can email us at referral@perio.ca or you can use our online referral form.
