We will schedule your appointment as promptly as possible. We will make every effort to be respectful of your time and ask in return that any appointment cancellation be accompanied by 24 hours notice. Thank you for your cooperation and consideration in this matter.

Treatment Plan:
 
In order to achieve sucessful periodontal therapy, it is important that you fully understand the condition of your oral health, the treatment that is necessary and the fees involved. We will thoroughly explain all of this to you prior to your treatment being initiated and provide you with a written copy of the treatment plan customized for your needs.

Fees:
 
Fees are due at the time service is provided.
For your convenience we accept Visa, MasterCard, Debit as well as cash and personal cheques.

Insurance Information:
 
Insurance is a contract between you, your employer and your insurance company. We file insurance claims as a courtesy to our patients. We will not become involved in disputes between you and your insurance regarding deductibles, copayments, covered charges and secondary insurance. We will assist you in any way possible to help with insurance claims. Predeterminations will be forwarded to the patient's insurance to eliminate any question as to coverage.
